Hormones change. Your strength doesn’t have to.
Perimenopause can bring new leaking, pressure, and discomfort. None of it is something you simply have to live with.
Perimenopause has a way of showing up in places nobody warned you about. You might notice leaking when you cough that never used to happen, more trips to the bathroom, waking at night to pee, a feeling of pressure, or sex that’s suddenly uncomfortable. These changes are common. They’re also very workable.
What’s changing
Estrogen helps keep the tissues of your bladder, urethra, vagina, and pelvic floor supple and well supported. As it shifts, those tissues can become thinner and less elastic. At the same time, muscle mass and bone density naturally begin to decline unless we give them a reason not to. Your pelvic floor feels all of it.
Symptoms worth bringing in
- Leaking with coughing, sneezing, or exercise
- Sudden urgency, frequent trips, or waking at night to pee
- Pressure or heaviness, which can be a sign of prolapse
- Dryness-related discomfort or pain with sex
- Losing confidence in the activities you love
How pelvic PT helps
We work on strength and coordination in your pelvic floor and core, bladder habits and retraining, and how you manage pressure when you lift. Strength training and bone-health exercise are part of the plan too, because this is a great time to build muscle, not lose it. If sex has become uncomfortable, we go at your pace, starting wherever feels safe.
This chapter can be a strong one. The work is the same as any strength training: steady, progressive, and built around your life.
Hormonal treatments, including local estrogen, are a conversation for your menopause provider. With your permission, I’m glad to coordinate with them so your care pulls in one direction.
This article is general education, not medical advice, and it doesn’t create a patient–provider relationship. For guidance about your own body, book a free 15-minute call — that’s what it’s for.
